On September 25, 2001, a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $3,000 was issued for a Severity Level III problem. The problem included two violations involving the failure to control and maintain constant surveillance of material in controlled or unrestricted areas and the failure to make surveys (i.e., evaluations of the radiological conditions and potential hazards incident to the use, transfer, release, disposal, or presence or radioactive material or other sources or radiation).
